About Holy Cross Physician Partners About Holy Cross Physician Partners Formed in 2011, Holy Cross Physician Partners (HCPP) is a clinically integrated health care provider network that unites community physicians and Holy Cross hospital in a care delivery team that promotes improvements in efficiency and health outcomes. At HCPP, we feel this approach is crucial to a sustainable, high quality health care delivery system that provides value to patients, employers and payers by delivering outstanding clinical care and outcomes while reducing inefficiencies, redundancies and their associated costs. Governance and leadership for Holy Cross Physician Partners is provided through physician dominated committees who oversee the negotiation of contracts, credentialing, recruitment and the performance improvement goals of the network. These committees are comprised of independent community physicians and physicians employed by Holy Cross Hospital. Our organization is led by an experienced executive team. Nationwide, health insurers have reported that the majority of their provider agreements will feature some level outcome-based reimbursement over the next 2 to 3 years. Medicare has already tied future physician compensation to quality reporting criteria. This has led many to believe that physicians will indeed have ‘no income without outcomes’ in the future. In our market, we have seen tremendous interest from employers and payers in our fundamentally different and more comprehensive approach to coordinating care across the health care continuum. Insurers are seeking organizations like HCPP that can coordinate care teams across inpatient and outpatient disciplines to ensure the right care is delivered at the right place at the right time—all at the right cost. HCPP has secured and will continue to seek health plan contracts that provide appropriate base-rate compensation to our participants, but also provide for additional compensation via performance-based bonuses and incentives. Physicians in the HCPP network determine the targeted outcome metrics as well as the manner in which performance bonus incentives are distributed to fellow HCPP providers. The HCPP management team is experienced in practice operations and understands the challenges physicians face every day. We work with our HCPP physicians to collect and share data in a sensible way and provide the support physicians need to succeed in a changing industry. To that end, physicians in our network have access to a secure quality reporting portal where they can view their own performance on objective quality criteria, identify patients who are lagging in outcome measures and document if and when gaps in care have been addressed. This portal is utilized by HCPP committees to monitor our performance under our health plan contracts. Our physicians have the security of knowing that their individual performance data is not controlled solely by the health plan. Today, hundreds of physicians in Broward and Palm Beach counties are part of Holy Cross Physician Partners. These physicians are united in the premise that better quality, higher patient satisfaction, and lower overall health care spending comes from a deliberate, coordinated effort between partner physicians to share clinical information with one another.
