Mission Statement The Senior Care Group Foundation provides philanthropic support for senior living programs and promotes community awareness of the services of Senior Care Group, Inc. Develop programs that provide financial assistance through charitable giving Senior Care Group, Inc., is a nonprofit 501(c)(3) organization that provides skilled nursing, rehabilitation services and home health care in four states. Services provided include 24-hour skilled nursing care; IV therapy; respiratory services; wound care; medication management; and physical, occupational and speech therapies. Diamond Home Health, our home health care division, provides home health services in Hillsborough, Pinellas, Polk, Pasco and Manatee Counties in the state of Florida. Transitions Rehabilitation provides therapy services at Senior Care Group's 6 skilled nursing facilities in Florida. Transitions works closely with Diamond Home Health to provide home health therapy services in the Tampa Bay community. nonprofit long-term care organization founded in 1983. We provide skilled nursing, rehabilitative and home health services. Transitions Rehabilitation provides inhouse therapy at all of our skilled nursing facilities, and outpatient therapy services at select locations.
