Please remember, if you are insured through an HMO or HMO Medicare replacement plan, you must have authorization from your primary care physician before visiting our office. We will send an itemized statement once we have heard from your insurance. You are responsible for your insurance deductible and annual co-payments. Quigley Eye Specialists also participates with accountable care organizations (ACO's) throughout Southwest, Florida. Quigley Eye Specialists works closely with local social agencies in providing needed eye care. We participate annually with the American Academy of Ophthalmology’s National Eye Care Project.
