PBACO’S INITIAL APPROACH TO DELIVERING ACCOUNTABLE CARE is by empowering the primary care physicians to act as BENEFICIARY ADVOCATES through building a relationship of trust, understanding and dependability. The goal is to get the beneficiary, who has open access to any primary care physician specialist, or hospital, to remain aligned closely with their PBACO physician. The physician will identify the unique healthcare needs of each beneficiary to create a personalized healthcare plan. The coordinated execution and evolution of this personalized plan will be the basis with which the physician will prove and demonstrate his/her dedication to the beneficiary’s health and well-being and deliver quality outcomes, appropriate utilization, and exceptionally improved beneficiary engagement. Aside from engaging the beneficiary to be involved in their care to the maximum extent possible, the personalized care plan will implement best practices based on evidence-based medicine; coordinate care among and between providers and facilities; use data to support continuous quality improvement; use community resources to the extent necessary to support and assure the beneficiary received the specific care they need at that time. The Centers for Medicare & Medicaid Services (CMS) has established a Medicare Shared Savings Program (Shared Savings Program) to facilitate coordination and cooperation among providers to improve the quality of care for Medicare Fee-For-Service (FFS) beneficiaries and reduce unnecessary costs. Increased coordination of care typically means increased communication between ACO health care providers, improved outcomes in overall patient health care, and reduced costs to Medicare. Patient engagement and engaging the patients’ families are key factors in improving patient health outcomes. Patient Engagement in an ACO has two primary goals: expand the doctor-patient relationship and educate the patient. ACO doctors attempt to share the decision-making process with their patients and well-informed patients are more likely to follow through with their joint treatment decision. Internal Medical Physician Dr. Irma Lopez was one of the first PBACO doctors to see the benefits of a new text message app with Jupiter hospital that allows her to receive daily reports on her patients being admitted and discharged from the hospital. This timely information allows her to communicate with her nurse practitioner immediately to schedule follow up appointments with these patients. In the past, this process could take two or more days. Cardiology Partners’ Duccio Baldari, MD, FACC, FSCAI, FESC, FSVM is board certified in internal medicine, cardiology, interventional cardiology and endovascular medicine. He has found that being a member of PBACO has enabled him to improve coordinated care for his patients. “Being part of PBACO allows my staff and me to provide a more comprehensive approach to the treatment of our patients,” said Dr. Baldari. Mudit Jain, M.D., F.A.C.E., E.C.N.U., C.D.E. is board certified in internal medicine as well as endocrinology, diabetes, and metabolism, and is among the few endocrinologists in the country to have received the Endocrine Certification in Neck Ultrasound (ECNU) by the American Association of Clinical Endocrinologists. As a Broward member of PBACO, Dr. Jain says his involvement will give patients a choice of a specialist located closer to south county residents. “I strive to provide, efficient, state of the art care in my specialty,” he said, “and believe being part of PBACO helps me stay abreast of ongoing changes in healthcare.
