Located in the heart of Jacksonville, Florida, we are positioned conveniently to serve patients in the Greater Jacksonville, Northern Florida, and Southwest Georgia regions and beyond. We look forward to helping you. When you first arrive, our office staff will greet you and check you in. They will ask you to complete paperwork. Some of this paperwork will help us take better care of you, some is required by insurance companies and the government. During the check in process we will make a copy of your insurance card, accept any copayments and ask if we can take a picture of you to attach to your electronic medical record file. This picture is very helpful in terms of patient safety, as it helps us link you to your records. This is especially helpful when we speak with you on the phone, consulting with your other doctors, and immediately before procedures. Should an abnormality be noted during the exam, a small intervention may follow. For example, if infection is noted in a sinus, a small sterile collection device can retrieve a sample for microbiologic analysis. This information helps determine the appropriate antibiotic. We would like to accept most major insurance plans. We have applied for, and are awaiting approval from, several. We are currently approved to participate in these plans below.
