Medication Therapy Management (MTM), such as those provided at our practice, have been shown to possibly save, on an annual basis, around $1,595. Medication Therapy Management (MTM) is a distinct service or group of services that optimize therapeutic outcomes for individual patients. It involves an interactive dialogue about the patient’s medications and conditions, a thorough assessment of any problems identified, and development of interventions to reduce the risk of medication-related problems, disease, and disease progression. This differs tremendously from the routine patient counseling that a pharmacist provides when a patient picks up an individual prescription medication. : The pharmacist keeps in touch with other healthcare providers, such as the doctor (or a number of doctors), bridging the gap and keeping track of every aspect of the patient's health. Multiple studies has been performed to demonstrate the positive outcomes that MTM provides both clinically and financially. Another example would include a CMS demonstration project in Connecticut which evaluated the impact of MTM services provided by nine pharmacists to 88 Medicaid patients over a 10-month period. Pharmacists were able to identify 917 medication related problems and resolved approximately 80% of them which resulted in annual savings of $ 1,595 per patient. Medication Health Center (MHC) is one of the first of its kind to deliver strictly MTM services. Under the direction of Ibukun Adetula, RPh, PharmD., MHC will not only educate, but also empower its patients to take an active role in managing their medications. We believe that not one profession can solve all the body’s ailments and that collaboration is key. MHC will collaborate with physicians and other healthcare professionals to optimize the medications that have been dispensed to our patients.
