Hospice of St. Francis is, today, Brevard’s longest-serving and only independent not-for-profit hospice. We are deeply grateful for the confidence Brevard residents have shown in us since 1977. We believe it is because, from our first day, we have our mission: care for families…support for loved ones. Everything we do, are, and believe at Hospice of St. Francis is found in our mission statement… Our Mission Uplifting the lives of our patients and their families is the mission of Hospice of St. Francis. Our team at Hospice of St. Francis cares for and supports patients and families, helping them achieve the most positive possible response to an emotionally and physically challenging time. Serving them uplifts their lives, and ours. In Fiscal Year 2016, Hospice of St. Francis achieved accomplishments in care and financial strength that enable us to reach more people throughout our community and look to the future with confidence. We hope that our 2016 Report to the Community will provide people with a better understanding of our caring approach to patients and families. This hospice has grown from our early days as a volunteer-only provider to today having a professional staff of over 100 and dozens of dedicated, highly trained and experienced volunteers. Hospice of St. Francis is, today, Brevard’s longest-serving and only independent not-for-profit hospice. We are deeply grateful for the confidence Brevard residents have shown in us since Emilie Sasko first had the vision to create Hospice of St. Francis. We believe it is because, from our first day, we have lived the mission to provide compassionate, quality care for the terminally ill, support for their loved ones and resources for community members faced with end-of-life issues. The mission of the Hospice of St. Francis Foundation is to provide compassionate, quality care for the terminally ill, support for their loved ones and resources for community members faced with end-of-life issues. It is because of support from you and other community members that we can provide these services to our community. Hospice is a specialized type of care, not a place. Hospice provides quality, compassionate care to individuals facing life-limiting illnesses. The care provided focuses on maximizing the quality of life based on an individual’s choices, so that the person may live life as fully and comfortably as possible, for as long as possible. I know we need more help to care for my mother but we are not sure if she is ready for hospice. What should we do? Call us. An intake professional will discuss your options with you, and if you are not ready for hospice right now, we can help you access other community resources that might help you. We often hear from physicians that they wait for their patient and their family to ask about hospice first. It is important to speak with your health care team about all options available to you for your care and this includes hospice. It is never too early to begin a conversation with your physician about hospice. If you are not comfortable with doing this, call Hospice of St. Francis, and we can help you to have this conversation with your physician. Where is hospice care provided? Hospice care is provided in many different locations: private homes, assisted living facilities, nursing homes and hospitals. Hospice of St. Francis also provides hospice in a free-standing No. Hospice of St. Francis values the relationship you have with your physician. The hospice physicians are available to partner with your doctor and the hospice team to provide effective pain and symptom management. You simply need to call us. Anyone can refer someone for hospice care. We have a team of professionals that are available to discuss your specific situation and help you decide if it is time for you to choose hospice. Who will provide the hospice care? Hospice care is provided by a team of professionals that include a Registered Nurse, Hospice Aide, Hospice Physician, Social Worker, Chaplain and Volunteers.
