October 1985, a Certificate of Need was awarded to Hospice of Citrus County and in August 1986, the first patient received services. 2006 a Certificate of Need was awarded to Hospice of the Nature Coast to provide services in 3A, which includes Alachua, Bradford, Columbia, Dixie, Gilchrist, Hamilton, Lafayette, Levy, Putnam, Suwannee, and Union counties. Admission into the Hospice of Citrus and the Nature Coast program is based on: Acceptance and understanding of the Hospice philosophy by the patient and family Services provided in Citrus, Levy, Columbia, Hamilton, Lafayette, Suwannee, Alachua, Bradford, Dixie, Union, Gilchrist and Putnam Services provided or arranged by Hospice of Citrus and the Nature Coast Respite provided by volunteers (no hands on care) Respite care in local skilled nursing facility as provided by Medicare/Medicaid Hospice benefit -Designation of Health Care SurrogateFive Wishes has become America’s most popular living will because it is written in everyday language and helps start and structure important conversations about care in times of serious illness. Comfort Blue was designed for members who have chosen hospice care and want to take advantage of the services offered by BCBSF’s personal case managers. Our case managers collaborate with the doctor, hospice provider and the member to: ensure shared decision-making with a full understanding of member goals and wishes, reduce unwanted and unnecessary hospital admissions and trips to the emergency room, and and select option 4. To help you learn more about this new program, we developed a brief five minute audio presentation.
