Just verify a network provider and present your membership card at the time of service. That's it! The Formulated Dental Plan is not insurance, so you don't have to worry about paperwork. Discounts are received immediately at time services are rendered. The minimum term for Formulated Dental Plan Membership is one (1) year. Membership fees are drafted monthly. We accept credit cards or electronic checking account drafts only. Please note this is not a health insurance policy and we do not make payments directly to medical services providers. You are obligated to pay for all health care services. You will receive discounts for medical services at certain health care providers who have contracted with the plan.
