Mission All of FHPCA’s Hospice Providers Members have a voting seat on our Board of Directors, and can name one Alternate that can vote in the Board Member’s stead. Our Executive Committee officers are Board Members that have been elected by the Board of Directors and consists of a President, Internal Vice President, External Vice President, Treasurer, and Secretary. The All employees of our Hospice Providers Members enjoy the privileges of FHPCA membership and can benefit from the discounted educational programs available. FHPCA encourages all members to become involved by joining one or more of our working committees Before joining Cornerstone Hospice as President and CEO in 2013, Chuck had more than 11 years of executive-level hospice experience. Chuck holds a Bachelor of Arts, magna cum laude, from Loyola University. He earned his Master of Arts at The George Washington University, and his Master of Business Administration at the University of Wisconsin-Madison. Chuck has also completed an extended internship in Clinical Pastoral Education at Baptist Hospital in Pensacola FL, and he is a Permanent Rana McClelleand, MBA, is the Regional Program Director of Florida operations for Compassionate Care Hospice, a large national hospice and palliative care provider. She was previously Vice President of Palliative Medical Associates, a national palliative care provider and a division of hospice and palliative care services for patients in need. She is an acknowledged expert in the hospice an palliative care industry whose many accomplishments include founding Compassionate Care Hospice’s Florida operations in 2010 and rapidly expanding to 3 programs in 6 counties by 2015. She serves on the board of the Florida Hospice and Palliative Care Association (FHPCA) and is currently the Internal Vice-Chair of the Executive Committee. A native of Huntsville, Alabama , she received her undergraduate degree at Auburn University and her MBA at Southeast University. The Patron Membership is designed for businesses engaged in the principle activities of hospice on the statewide level. This includes vendors of all types, consultants, agents, pharmaceutical companies and all organizations serving the end of life continuum. The Patron Member receives an array of benefits that will strengthen the member-to-member relationship with between Patron and hospice executives. The goal is to facilitate good professional relations between those with solutions and those with the need before a business transaction takes place. This is accomplished through three or more Patron Invitational with Hospice CEOs, strategic discussions and planning, and continual opportunities to expand relations. Associate members are typically businesses who provide goods or services Florida hospice programs may need. This membership is one of the most important ways any business can show support of hospice and end of life initiatives. At FHPCA, we encourage our Patron and Corporate-Patron Members to participate in committees, where appropriate, to work alongside hospice leaders and to use their talents to further FHPCA’s mission for quality hospice in Florida. With hospice, care priorities shift to support patients and the care givers with end-of-life decision-making to reach the best quality possible for each day. Hospice provides quality care through a team approach and offers a means to achieve what people say they really want at the end of life: The option to remain in their own home, surrounded by loved ones. Help for the family. The hospice or the attending physician will determine what levels of care is needed using standardized criteria. Not every level of hospice provides 24-hour care. The goal of hospice is to empower the family, by providing the needed support. Not all patients use or need all levels of care. The hospice team and the attending physician determine what levels of care are needed by each patient. : The patient receives hospice services at home or in a home-like setting, such as nursing home, assisted living facility or a hospice residential facility. The family provides the primary care to the patient with the assistance of the hospice team. : Skilled nursing services that are provided in the patient’s place of residence to help during a crisis period. : This care is provided in a facility (hospital, nursing facility, or hospice freestanding inpatient facility) for symptoms or a crisis that cannot be managed in the patient’s residence. Inpatient care is provided for a limited period of time, as determined by the physician and the hospice team. : This service is provided in a facility (hospital, nursing facility, or hospice freestanding inpatient facility) and is designed to give caregivers a rest. Respite care is up to five days and nights at a time. This service is often used to provide a break so that caregivers can participate in other family activities, such as holiday celebrations, or just to relieve a tired caregiver for a few days.
