Direct Primary Care (DPC) is a new approach to paying for basic health care services. In most cases, payment for basic care is made directly to the primary care clinic and not through an insurance company. Direct Primary Care is not insurance. DPC provides access to primary medical services on a per month, membership basis. DPC may be supplemental to traditional insurance programs. For complete information, give us a call or bring us up to your licensed broker or insurance consultant. Insurance will not be billed for services provided by the Retainer Medical Agreement. The practice provides only the limited scope of primary care services specified in the Retainer Medical Agreement. The patient must pay for all services not included in the Retainer Medical Agreement. DPC provides only the limited scope of primary care services specified in the retainer medical agreement; Direct Primary Care Agreements with recurring dues may be canceled at any time and for any reason. You can cancel your membership by providing written notice to us at PO Box 453 Camas, WA 98607 or through our website. We recommend that you mail the cancellation notice by certified mail and keep a record for your files. Or, you may deliver the notice directly to the clinic manager at your membership clinic. A cancellation postmarked at least 5 business days prior to your next billing date should result in no further recurring billing. If less than 5 business days, you may be billed one more time. If this occurs, EverMed DPC will refund the additional billing. To ensure that we have accurate information about the account being closed, we recommend you print and use the online form.
