Discovery at Home is comprised of a team of professionals with in senior living personal care. We’re always nearby, including our senior management team, accessible to our clients and their families, and have direct authority to make decisions right where we are. Discovery at Home is with a Discovery at Home registered nurse case manager or home health aide at their side. Our home health team can be the primary and exclusive provider of services, or we can work along with other family members as caregivers.
