The CHP Case Management Program is a service offered to all CHP members. Our nurse case managers can offer assistance to members and their families who are facing a serious illness or injury and need help navigating the sometimes complex systems of medical care. Clinical services on Amwell are provided by Online Care Group - the nation's first and largest primary care group devoted to telehealth. Physicians on Amwell average 15 years experience in primary and urgent care, are U.S. Board Certified, licensed and credentialed. You have the ability to view each physician profile so you can see their education and practice experience. Capital Health Plan knows that the way we conduct our business offers us the opportunity to earn our customer's respect and trust. Our customers are our reason for being. To that end, we believe our customers have the right to privacy and that their non-public personal, financial and health information should be kept confidential. CHP safeguards oral, written, and electronic information. Confidentiality agreements are signed with any contracted parties that receive a member's confidential personal, financial, or health information. Access to this information is restricted to those employees who use the information to provide services for members. To that end, we believe our members have the right to privacy and that their non-public personal, financial and health information should be kept confidential. This Privacy Notice is provided to help you better understand how we protect your non-public personal, financial and health information. If you are one of our members you can call Member Services at 850-523-7441 or 1-877-247-6512 (TYY 850-383-3534 or 1-877-870-8943) 8:00 am to 8:00 pm,  seven days a week, October 1 - February 14; 8:00 am - 8:00 pm Monday - Friday,  February 15 - September 30.  State of Florida members call 1-877-392-1532, 7:00 am - 8:00 pm.  Generally, the information includes your name, address, phone number, social security number, date of birth, age, sex, height and weight, occupation, health habits, and general medical information. It could also include accident and injury dates. Most of this information is provided by you. Information you provided us, or an affiliate, on an application form for health plan coverage; Information your physician or other health care practitioner provided us; Information your employer provides us; We may share any of the information we collect about you (as described above) with our affiliates, namely our health business partners. We may disclose any of the information we collect about you to other parties, including vendors and governmental agencies retained to audit medical records and billings. However, the type and the amount of information we share with others is limited to what is necessary to implement and administer the health plan you have with us, or as otherwise permitted or required by law. We may disclose any of the information we collect about you (as described above) to companies that perform other services on our behalf, including administrative services. We maintain physical, electronic, and procedural safeguards to protect your personal information. We access and use your personal information to the extent necessary to administer the health plan services you are entitled to. We establish confidentiality agreements with contracted parties that receive nonpublic personal financial and health information about you. We restrict access to your non-public personal, financial and health information to those employees who need to know that information to administer the product or service you purchased from us. Capital Health Plan has had a Compliance Officer since November 1999, and a Compliance Committee chaired by that officer since December 1999. This committee meets regularly throughout the year. The meetings are announced in the employee newsletter.Additional information regarding the dates and responsibilities of the Compliance Officer are found in the "Beacon Program for Compliance". detection, correction and prevention element for CHP Compliance is reflected in the "Fraud, Waste and Abuse Awareness and Reporting Policy" found on our intranet public website in the Beacon Compliance reference. This policy is a part of workforce orientation during initial on-boarding at CHP.  Fraud, Waste and Abuse concepts are part of initial and annual computer based training for all staff in addition to Fraud Awareness training provided by the Florida Blue Special Investigations Unit presentations. This FWA policy is also communicated to our Network and downstream entities via various communications including remits, Network News and internet postings and annual attestation requests. This summary is not intended to be all inclusive of the entire scope of Capital Health Plan's Compliance program, but is designed to present a high level overview of the Compliance elements currently in place. In accordance with the Identify Theft Red Flags and Address Discrepancies, through the Federal Trade Commission issued 11/1/2008 Capital Health Plan strives: 1) to prevent the intentional or inadvertent misuse of member names, identities, medical records; 2) to report criminal activity relating to identity theft and theft of services to appropriate authorities; and 3) to take steps to correct and/or prevent further harm to any person whose name or other identifying information is used unlawfully or inappropriately. The Red Flag means a pattern, practice, or specific activity that indicates the possible existence of identity theft. The Red Flag regulations require healthcare entities to have a written Identity Theft Prevention Program that is designed to detect, prevent and mitigate identity theft. The purpose of this policy is to set forth the guidelines for Capital Health Plan management and staff to use as an on-going effort to detect, prevent and mitigate identity theft.
