We Provide Family and Geriatric Care Management… Geriatric Care Managers (GCM) are professionals who have specialized training in gerontology, social work, nursing and or counseling. A Geriatric Care Manager’s role is to provide older individuals and their families, the assistance they need to navigate and manage their long term needs. The focus of care management is on the individual and the family as a whole. Individual’s preferences, strengths, weaknesses and needs are assessed and a plan is developed to help maintain a person’s autonomy, dignity and safety. Care managers can provide advocacy and education. They act as a liaison to families at a distance. Care managers are there in time of need and crisis. They can screen, arrange and monitor in-home help or other services the client may need. Geriatric Care Managers work as a team with many other professionals such as doctors, financial planners, attorneys, trust officers, guardians, and many other aging specialists. Mary Beth Johns RN, MSN is the Managing Member for Aging and Family Solutions, a geriatric care management company in Collier and Lee Counties with an office located in Naples. As one of the company owners/investors, she has helped establish Aging and Family Solutions in the area with her husband, Charles. A combined Masters in Nursing and Business has been useful in the development of new business relationships, creating our business model and understanding the psychosocial and medical needs of clients. With her leadership, Aging and Family Solutions will continue to grow providing employment opportunities and has allowed many to “age in place”, providing quality of geriatric care management to our community. Chuck Johns has over forty years of senior management experience in hospital and health care systems. From a career that began with the U.S. Public Health Service following graduation from Cornell University’s Sloan Institute of Hospital Administration with an MBA, Charles A. Johns has become a leader in the healthcare industry, rising to CEO positions with a community hospital and integrated healthcare system with 5,000 employees and 1,200 physicians. He is now applying his senior-level management experience to assisting with the ownership and management of healthcare industry companies such as Aging and Family Solutions, LLC, our geriatric care management company.
